Legal name, DBA, mailing address, website and FEIN.

Name, title, direct phone and email for the agency principal.

Licensing
Active resident P&C producer or agency license number and states.

E&O certificate
Current certificate — $1M minimum coverage recommended.

Completed W-9
Current IRS Form W-9 (Rev. March 2024), signed for the agency, for commission setup.
